Urgent information about clinical training

Please take out print outs of these two papers. The first one is to be
filled by you in which u have to write down your details like name ,
hospital posted , duration of training etc. so that we can keep a record of
it and the second attachemnt i.e. two pages in another attachment need
to be taken as print out and filled by you during ur taining about the
patients details,
For Eg.
u can write down the patient name as case 1 in case u dont want to reval
their identity etc, then u must fill the history of the patient,
diagnosis n treatment given. Like this all the relevant information
whichever has been asked in the format.These two papers then need to
get xeroxed on a single A4 sheet i.e. at the front n then d back so
that u dont have to carry a big pile of paper at the end.
You need to fill for about 25 patients during your tenure in the
training. This will help u to understand better abt d cases plus would
help u to make case sheets at the end of the session as u used to do
earlier. If u find anything important which can add to ur knowledge
then u can also attch a blank pare so as remember the facts later on.
At the end u need to get these papers spiral bound n please bring them
to the college whenever u end up with ur taining.
