Dear DPMs,

 

                  Extremely happy for quick response,  and  identifying one staff at PHC who will be resposible for data entry of HMIS and MCTS. Please also find the MD(NRHM) letter on the same subject which was mailed to you couple of minutes back.

 

MD(NRHM) has instructed me  to collect  the latest position of Hardware status in different PHCs of the state. Since you have already provided certain details, the same is attached to this mail so that it will be easy for you to co-ordinate and get the information. Please kindly fill up last 3 columns and bring the details while coming for HRMS  meeting on 8th of this month.

 

All DPMs are kindly requested to provide the information pertaining to your district deleting the rest. Send the details over mail and bring soft copy on your pen drive while coming for HRMS meeting.

 

Please treat the matter as most urgent.

 

Failing to bring the information will be viewed very seriously
