The FORM B data entry is still not completed. On an average, Only about 50% is completed as on today. Secretary HFW has viewed this matter very seriously. During the Video conference, the DHOs had promised to completed the data entry by the end of June. It is regretted that the progress is still not encouraging. Secretary has decided to call a meeting of all the DHO/ DPMOs of those districts who do not complete the data entry by 30^th June 2011.

Kindly take the matter very seriously. Hope you will complete the data entry within the deadline.
