http://www.rtiindia.org/forum/20076-all-about-form-17a-used-during-elections.html clarifies that “AS SUCH THERE IS NO FORM 17 A which can be handed over to a elector for his use in refusing the vote” and also that “The Presiding Officer showed his ignorance and shuffled all sorts of forms and papers supplied to him by election Commission”

http://eci.nic.in/eci_main/CurrentElections/ECI_Instructions/ins_121007b.pdf is clear about Form 17A, but the officials in polling booth seem to be confused.

Form 17A is register of electors, where we have to enter ‘none of the above’ choice.
