This is my first winter w. the bees. 
Should I get a shim with upper entrance?
How to handle moisture buildup?

Yes, put some popsicle sticks up there at least, to let a little bit of air flow. This was recently confirmed in a wintering presentation at Mass Bee by Gary Reuter from very cold UMN.
