Windows cannot access the specified device..

Hi,

After I recompiled my program then I got "Windows cannot access the
specified device, path, or file. You may not have the appropriate
permissions to access the item". I know there many I/O routine (open
files) in my program but how do I know the problem maker?

How come this error occurs? Why Windows already know my I/O routine
even my program uses mutex to avoid from opening twice.
