Hey,
there have been quite a numebr of problems with this, but finally they are there -- margin notes that flow along with the text, never overlap, yet try to be as close as possible to the reference to them in the original text.
To try it out, go to page 5 on the following url and put the cursor at the start of the first partagrpah there. then hit enter several times until the entire page is empty. Notice how the amrgin notes are moving around, accordingly: http://sourcefabric.github.io/BookJS/test7.html
