Hi Ashley: Please print the attached document. Page 2 is a proof of loss form which must be signed by an authorized person representing the condo association, and a witness signature as well. Once complete, please forward the document to me via scan/email or photo/text and payment will be issued at the current undisputed actual cash value amount of $59,297.95.

We received approval from Arbella Protection Insurance for our repair estimate which is attached. In summary, the approved settlement is as follows:

Please note, the unit owner has a contractor and presented a price of $71,873.50, to which we do not currently agree. We are extending our approved estimate as an offer to the association as a fair value for the repairs.
This estimate does NOT include covered repairs to the sprinkler system; we await invoice(s) from the unit owner for that work which will be added in the future.
