Hello Boston Latin Academy alumni,


The REaDY Initiative (Reproductive Empowerment and Decision Making for  
Young Adults) is a coalition of Massachusetts health care providers,  
advocates, and researchers looking at the impact of health care reform  
in Massachusetts.

We are producing a Guide to make sure that young adults aged 18 to 26  
have the resources they need to lead healthy sexual and reproductive  
lives.

That’s why we need your honest opinions on the two different design  
options for the Guide. The Guide will be available online and can be  
viewed like an electronic book with interactive links to allow you to  
easily read the sections that most interest you.

 to respond to our brief survey. You will see the two different  
design options, followed by six simple questions. We’d like your  
opinion about the style and look & feel of each one, including which  
one is your favorite. Please type your responses to each question in  
the space provided and click ‘finish.’  This should take you no more  
than 10 minutes, but we need your feedback by midnight on September  
13. That’s it!

To thank you for participating, we will be holding a drawing for a  
$100 gift certificate to Amazon.com.  When you complete the survey,  
you will be asked for your name and email address.  One lucky winner  
will be randomly chosen from all of those who participate. We  
guarantee that your name and email will only be used for this  
drawing.  They will not be associated with your answers on the survey  
or traded or sold to anyone else.
