Summary	:	In view of the implementation of GST effective 1 April 2015, the following forms will be revised to incorporate the new fees inclusive of 6% GST and must be used on the stipulated date onwards.   a)      MEPS Direct Debit Authorization Form b)      Trust Nomination Service Application Form c)      Transfer form   The forms will be made available in branches by 23 March 2015
