Dear Medlibbers,
An OB in my hospital wants me to subscribe to a number of OBGYN
journals and to house them up on the OB floor - not in the library.
Has anyone had this requested of them? I feel this would set a bad
precedent and do not want to oblige, however, I may be "encouraged" to.
I would appreciate any comments!

Our policy (hospital as well as Library) is that if an item - books and
journals included - is for use on a unit or in a department, the item is
charged to the department. Each department/unit has a budget that includes
a line items for subscriptions, so there's no excuse. And I make sure that
that's where those purchases turn up, since I handle global (hospitalwide)
book/journal/av material purchases orders and enter them into the materiel
management database as I create the POs. Each department gets a monthly
printout of purchases (and a quarterly report) from which they can plan
their next year's budget. The library is charged only for those titles
which remain in the library and are circulated to the hospital community at
large.

In some cases librarians may be missing a "golden" opportunity by not
ordering/processing journals or books for departments. Just offer this
service to all departments for a "fee." So much to order for them, so
much more if they want journals checked in/claimed by library staff, so
much for processing/cataloging books, etc. This serves two purposes (at
least), generating additional income for the library, and increasing the
library's service visibility which helps us down the road to
INdispensibility. I vote with Naomi Broering whose keynote address at
the recent Southern Chapter meeting suggested a "yes,yes,yes" attitude
when asked to take on increased responsibilities. The librarian or
library which can turn this to very positive advantage will be our
success stories.

Dear Colleague,
I had the exact same experience. The Chairman of the OB Gyn department wanted
me to remove all the ob books and journals and house them in the OB lounge
because"no one else used them." I refused. He became very upset. We discussed
the idea at Library committee meetings and even ended up at the Executive
committee. My position was that "library" means all topics accessible in one
place to all one's customers. The library committee agreed to purchase a
duplicate set of core books for the lounge; the chairman declined. We ended up
with a compromise...I take the previous month's issues of four ob journals to
the OB lounge. Any books, audio or video tapes we discard from the library go
to the lounge.
I really dug in on this because I do not have the staff to maintain separate
collections in Ob/ then surgery/ then anesthesiology and who knows how many othe
r places once tghe precedent was set. It is also a bad idea because you will
never know what your circulation on the books is; should you purchase a new
edition or not? Also, who would watch to see the items were returned to the
site? Losses would be high.
We have used our "system" three years now and the Ob's seem satisfied. All my
physiicans also have badge access to the library - so they can come in and use
all the new materials whenever they like.

The same thing happened here several years ago, when the ER requested
that a small reference collection be placed in that department by the
library. This was one time I was glad to have a library committee. The
issue was discussed and the committee decided NOT to furnish books for
hospital departments, for the following reasons:

1. The library exists to provide a readily available, centralized
resource for the hospital. Similar requests could come from other
departments; the library cannot put books in one area and refuse to put
them in other areas. With a finite amount of money available, such
decentralization of the library would begin to diminish the main collection.

2. Most physicians (including hospital-based such as Xray, lab, etc.)
routinely purchase books for use in their own office or practice
situation AND contribute to the library fund. Every department also has
a budget for necessary books and journals; it is up to them to justify
reasonable purchases.

Again, this is when it's helpful to have a committee, not only to help
make a potentially unpopular decision, but to support you afterwards.

In this day and age, suggest an hospital wide online system, and the
purchase of journals on CD to network to all. It will cost more, so that
budget support is needed. I did not check into the reality of such a
virtual approach for the specific OB/GYN dept, but I am also sure that a
sub-set of medline plus full text books and selected journals is
available in CD format fir OB/GYN as it is for urology, ortho, etc.

Here's where a collection development policy can really help you! If you
have already spelled out in the policy that materials are for the use of the
whole hospital and that specialized materials relevant only to one
department should be purchased out of departmental funds, you can easily
cite that policy. I can understand that OB GYN docs are often waiting
around but so are other docs--surgeons, ER doctors, neonatologists, house
doctors. If you do this for one, EVERY area will expect the same (as a
prestige thing if nothing more).

Also, the idea of housing previous issues of journals in a department is a
very bad idea--presumably other doctors--internists, etc.--might want an
article from one of those issues and retrieval will definitely be difficult
if not impossible. We ran into this; radiology journals were not subscribed
to because the radiology department kept an extensive collection. Retrieval
was impossible so we finally bought our own subscriptions to AJR and
Radiology so that they were available to non-radiologists.
