MLA question... combined Notes & Bibliography

Good morning-
I am not sure this is the best group to post to, but ask your
patience, and hope you have some wisdom for me.

I am helping to edit a manuscript that is in very rough form at
present.
My author would like to combine the Notes & Bibliography page into one
document, as is used in David Brooks' new book, published by Random
House (2011), THE SOCIAL ANIMAL.

I have been using MLA Style throughout the project, and now, with this
request, I want to be certain that I am not creating a works cited
page that will be rejected due to style inconsistencies. I see no
examples of such a page online, and would appreciate guidance.

Is this an acceptable format?
Do you know of online examples of acceptable combined Notes &
Bibliography?
Am I worrying about nothing?
