Dear Birth Center Supporters,

The Birth Center rules for the Health Facilities Services and Review board became effective 1/31/12. 

 

The applications for Birth Center licensure are  available on line!

There are TWO applications that must be completed:  One for IDPH and one for IHFSRB.

There will be a special meeting with representatives from the HFSRB and IDPH to review both application forms

For those unable to attend, Health and Medicine plans to do a summary of the discussion.
