Boone Health Connection will automatically provide a messaging link to Boone providers that you have visited (physically or via tele-health), since the Boone Health go-live date of April 1, 2021. However, if you have not seen your provider on or after that date, the patient portal system does not recognize your relationship with your provider yet. Please contact the Boone Health Connection Portal Team for assistance in generating a messaging link for all your Boone Health providers.

We take great care to keep our health information private and secure using the latest encryption technology. Access to information is controlled through personal user IDs, passwords and security questions.
For patients, 18yrs or older (including; spouse, parent, etc): If you do not have an email address or internet access and/or would like to authorize another party to view your protected health information (medical records) via the Patient Portal, please complete the Proxy Authorization Form or contact the HIM department to obtain the form.
In the MyChart by Epic patient portal, you will find many of your test results and clinical notes. If there is a record you are looking for that is not available in your MyChart, you can contact Health Records to request that record, for a fee. Call 905-521-2100 and ask for the Health Records department at the site you visited for your appointment.
ConnectMyHealth is also an online patient tool, meant for patients who receive care from more than one organization in the south-west region of Ontario, for instance hospitals in Hamilton, Burlington, Brantford, Niagara and London. This tool is replacing the regional Sunnybrook MyChart portal for the south-west region. ConnectMyHealth and regional health care tools are managed by the Greater Hamilton Health Network and the Ontario government. Hamilton Health Sciences does not manage regional tools.

7. Founder and CEO of Epic Judy Faulkner unveiled a new data research initiative and software during the Epic User Group annual meeting on Aug. 27. She highlighted the Cosmos program, which is designed to mine data from millions of patient medical records to improve research into treatments. The program gathers de-identified patient data from 8 million patients at nine health systems, and 31 more organizations have signed on to participate. The company also announced new products focused on letting physicians write shorter notes and voice recognition software.
