phd registration

Can any one help me in getting the information related to phd registration forms in the field of Biotechnology or biochemistry or botany.
I am having one international research paper and other communicated but iam very much upset becoz iam not able to apply for the phd registrtion due to unavailability of forms . DO anyone can provide me some of the mail id of university professer's  to whome i can contact for the registration. Iam working in NBRI Lucknow with3 yrs experience.

Thanx for the help, well i don't have much knowledge how to apply abroad and what are the universities which offer scholarship in Phd, and even i don't have GRE also, if you help me or guide me for the abroad universities ill try to apply.
