The wide expanse of Hemolytic anemias can go beyond just red cells, and can include Hematopoietic Stem Cells as well. One such entity is Paroxyxsmal Nocturnal Hemoglobinuria. PNH is an acquired, clonal hematopoietic stem cell disorder characterized by complement mediated persistent intravascular hemolysis, pancytopenia and a propensity to venous thrombosis. This triad makes it a unique disorder and its diagnosis equally challenging as its frequently associated with aplastic anemia and in some cases, MDS. Laboratory diagnosis is confirmatory and relies on Flowcytometry. The treatment, over the time has revolutionized with advent of targeted therapies and exploring transplant options in these patients.
