They are currently hiring for a Public Health Program Director 1, a full-time role focused on leading the implementation of Tennessee’s new Health-Related Social Needs (HRSNs) initiative in partnership with TennCare.
This one-year position (with the possibility of renewal) is primarily remote, though occasional in-person attendance at meetings and trainings in Nashville is expected.
The role will report to the Family Health and Wellness Quality Improvement Director and will serve as the day-to-day lead for this initiative, working with programs such as CHANT, Evidence-Based Home Visiting, and Viral Hepatitis.
Responsibilities include project oversight, stakeholder collaboration, training development, data monitoring, and facilitating adoption of the FindHelp digital referral platform to improve coordination of care across agencies and community organizations.
