Project intake form?

At the CfASummit, I was exposed to Code for Boston's and Open Oakland's project / problem intake form. I thought it was a genius idea and adapted it for potential use within BetaNYC.

Questions:
 - Would you use it?
 - Does this form meet your needs? 
 - What would you add / subtract from it?

I think we need a much clearer definition of what is an "official" betaNYC project.  Most of the things we work on grow organically, and there needs to be some incentive for filling out a form (money, resources, help from the developer pool, etc).  

I have told you this before, but I'll reiterate for the group... I think there should be a clear delineation between "official BetaNYC Projects" run by the organization and "community projects" run by individuals/groups who are affiliated with BetaNYC.  Anyone else have thoughts on this?
