Cafeteria questionnaire

Hi Folks,

Back in June some of us met with Jennifer Chin, new Food Services
director and she encouraged us to survey the whole school about making
changes to the cafeteria, like eliminating fast food, for example.
She wants to see that there is interest in change beyond this
committee.

As we learned at the meeting, the parents at Franklin surveyed their
population in January, and based on the results of the survey, the
cafeteria stopped serving fast food (i.e. McDonalds, Domino's, etc.)
within a month.

Based on the quick turn around, I'm hopeful that we can implement
change quickly based on the results of our own survey. I've taken the
liberty of starting a draft survey below using the Franklin survey as
a guide. Please review and make changes/suggestions. I hope we can
send this out in the Thursday folder (or by e-mail!!) by the end of
this month. I'll check with Wendy to find out what portion of the
school population is accessible by e-mail. I can also help translate
into Spanish. We can request help to translate into Armenian.
