DISCHARGE REFERRAL COORDINATOR - WHATCOM ALLIANCE FOR HEALTHCARE ACCESS
Under the IMPACT Project Lead Coach and matrixed to the discharge referral department at PeaceHealth St. Joseph Medical Center, the Discharge referral coordinator will oversee day-to-day operations of referrals of eligible Medicare fee-for service beneficiaries into IMPACT. IMPACT is a project funded by the Center for Medicare and Medicaid Services aimed to coach Medicare beneficiaries during their transition from the hospital setting to other care settings.
