Belfast Ultimate Membership Form 2016

Hi everybody,

Please find attached the adult membership form for 2016. All members will need to fill out a membership form for 2016, and organise payments for fees. Ideally, this form should have gone out before the end of December and you should have already set-up all your payments. Unfortunately the EGM failed and things got busy around Christmas for everyone. We're very sorry for the delay.

If you can get the forms filled in and returned, and the payments set-up fairly soon-ish, the committee would be very grateful. If you're setting up monthly payments, remember to pay for January too.

There's not much change from previous years

• Indoor fees for full members have gone down
• Indoor fees for affiliate members have gone up
• Tournament fees have gone up for everyone.

If you're paying by bank transfer(s), Dave would like you to use the new Co-op bank account (details on the membership form)

You only need to return the last 2 pages of the form. Therefore, you only need to print out the last 2 sheets. If you have difficultly selecting that as a print option, you can use this file, which is just the return forms bit of the membership form (i.e. the last 2 pages)
