Family Income Verification Forms

Families:

Each year CPS asks every parent to complete an Income Verification Form. With family income information, we can apply for State and Federal grants.  Many years Belding has qualified for up to half a million dollars of additional funding.

Please fill out one of these forms, no matter your income level. I complete a form every year for my daughter's school.

I am sending you a form in the US family. A pdf version is here. You can return this to Belding by US mail, by scan, by coming in person (7 am - 3:15 pm) or you can use our new drop box locking mailbox by the doorbell at the main door (door #2).
