The Friends and Family Test (FFT) is an important feedback tool that supports the fundamental principle that people who use NHS services should have the opportunity to provide feedback on their experience. Listening to the views of patients and staff helps identify what is working well, what can be improved and how.

The FFT asks people if they would recommend the services they have used and offers a range of responses. From April 2020, a new question has replaced the original FFT question about whether people would recommend the service they used to their friends and family. There will be delays in some services being able to implement the new guidance in full due to the impact of the coronavirus on services.

The replacement question invites feedback on the overall experience of using the service. When combined with supplementary follow-up questions, the FFT provides a mechanism to highlight both good and poor patient experience. This kind of feedback is vital in transforming NHS services and supporting patient choice.

After being launched in April 2013, the FFT was rolled out in phases to most NHS-funded services in England over a two-year period, and now gives all patients, users of services, their carers and loved ones the opportunity to leave feedback on their care and treatment.

During 2018/19, we carried out a development project to improve the FFT and make it a more useful tool for driving service improvement. This resulted in a number of revisions to the way it works, set out in revised guidance for services, and these changes are effective from 1 April 2020, though implementation in many services will be delayed due to pressures from the coronavirus response across the NHS,

The feedback gathered through the FFT is being used in NHS organisations across the country to stimulate local improvement and empower staff to carry out the sorts of changes that make a real difference to patients and their care. You can see examples of this in our FFT case studies.

While the results are not statistically comparable against other organisations because of the various data collection methods, FFT provides a broad measure of patient experience that can be used alongside other data to inform service improvement and patient choice.

Since April 2014, all NHS trusts providing acute, community, ambulance and mental health services in England have also been required to implement the FFT for staff. Guidance to support organisations administering the Staff FFT is published on the Staff Friends and Family Test page.

The Medicare Annual Wellness visit was initiated in January 2011 as part of the Affordable Care Act. The yearly Medicare benefit includes the creation of a personalized prevention plan and detection of possible cognitive impairment.

The Alzheimer's Association convened a group of practicing expert clinicians to make consensus recommendations for an effective, practical and easy process for detecting cognitive impairment in the primary care setting.

The resulting algorithm (PDF) incorporates patient history, clinician observations, and concerns expressed by the patient, family or caregiver. Because the use of a cognitive assessment instrument can improve detection of dementia in primary care settings1, the group identified several brief tools to evaluate cognition, all of which can be administered in five minutes or less by a physician or other trained staff.

These cognitive screening tools are used to identify individuals who may need additional evaluation. No one tool is recognized as the best brief assessment to determine if a full dementia evaluation is needed. However, the expert workgroup identified several instruments suited for use in primary care based on the following:

A growing area of research is the development of medical devices and technologies to help physicians evaluate cognition and function. Among these are medical devices or digital technologies including computerized testing software; these have several advantages, including giving tests exactly the same way each time.

Digital computerized assessments are designed to measure an individual's performance on a variety of cognitive or functional tasks and may be appropriate for administration in randomized clinical trials or clinical settings, when lengthier standard assessments are not practical.

The U.S. Food and Drug Administration (FDA) has cleared several digital cognitive testing tools for marketing:

The FDA has also cleared a medical device called Cognision. This device is a headset with electrodes that are affixed to the scalp to measure electrical activity in the brain responsible for cognitive function.

In addition to assessing mental status, the doctor will evaluate a person's sense of well-being to detect depression or other mood disorders that can cause memory problems, loss of interest in life, and other symptoms that can overlap with dementia.

The following videos show a brief cognitive assessment and a peer-to-peer discussion of important aspects of assessing cognition and disclosing an Alzheimer's disease diagnosis during a primary care visit. The videos, part of a three-part instructional series developed for primary care physicians, use an actual primary care physician and patient living with Alzheimer's disease. Psychologists administer tests and assessments for a wide variety of reasons. Children who are experiencing difficulty in school, for example, may undergo aptitude testing or tests for learning disabilities. Tests for skills such as dexterity, reaction time, and memory can help a neuropsychologist diagnose conditions such as brain injuries or dementia.

If a person is having problems at work or school, or in personal relationships, tests can help a psychologist understand whether the person might have issues with anger management or interpersonal skills, or certain personality traits that contribute to the problem. Other tests evaluate whether patients are experiencing emotional disorders such as anxiety or depression.

A psychological assessment can include numerous components such as norm-referenced psychological tests, informal tests and surveys, interview information, school or medical records, medical evaluation, and observational data. A psychologist determines what information to use based on the specific questions being asked. For example, assessments can be used to determine if a person has a learning disorder, is competent to stand trial, or has a traumatic brain injury. They can also be used to determine if a person would be a good manager or how well they may work with a team.

Psychological tests are not one-size-fits-all. Psychologists pick and choose a specific set of assessments and tests for each individual patient. And not just anyone can perform a psychological evaluation. Licensed clinical psychologists are expertly trained to administer assessments and tests and interpret the results.

A comprehensive psychiatric evaluation may be needed to diagnose emotional, behavioral, or developmental disorders. An evaluation of a child, adolescent, or adult is made based on behaviors present and in relation to physical, genetic, environmental, social, cognitive (thinking), emotional, and educational parts that may be affected as a result of these behaviors.

Many times, families, spouses, teachers, or friends are the first to suspect that their loved one is challenged by feelings, behaviors, or environmental conditions that cause him or her to act disruptive, rebellious, or sad. This may include problems with relationships with friends or family members, work, school, sleeping, eating, substance abuse, emotional expression, development, coping, attentiveness, and responsiveness. It's important for families who suspect a problem in any of these areas to seek treatment as soon as possible. Treatment for mental health disorders is available.
