How long was it running before the issue showed up?

I have a beaglebone Rev A6. Up until a few days ago when I plugged it into my computer's USB port, it used to show up as a drive containing the readme.htm file and other folders. But recently it is showing up as Unknown device. Things I checked are:

1. Plugged it in another friend's computer and I get the same message. Started troubleshooter and I get "Windows has stopped this device because it has reported problems. (Code 43)"
2. Plugged it into a PC with Ubuntu 12.04 and even there it doesn't show up.
3. Removed the SD card and inserted it via card reader and I can see the readme.htm and other folders intact
4. I can only see the Dual RS232-HS device (that too with a single port, the second port would usually be the one which I used for RS232 over USB) in devices and printers tab. The other icon is unknown device with exclamation mark.
5. I can still SSH into the device using putty (Beaglebone connected to router). I can navigate around when I am connected through putty, which means the Linux OS is working properly.

Any suuggestions?

Hi Gerald,

I had used the device for a week or so. Max continuous uptime was 6hours. Today I plugged it in another USB port and the Dual-RS232-HS port was detected and I was able to use the serial over USB. However the unknown device icon was still there in printers and devices.

I flashed the SD card with the image file and the beaglebone started working again.
