Dear FR development team

With great appreciation of your hard works for developing the FR GC8 both for the TB and the HIV proposal, I put few comments in the narratives for both of the FR proposals (attached). However, some key points are below:

For TB- 

The FR focused it services of early diagnosis with an integrated approach with PHC services using public facilities. But the urban population has not been addressed adequately where the TB incidents, new cases are higher than rural areas.  Form where the urban population will be accessed to PHC services? There is no as such PHC services in the urban areas like community clinics at rural areas. The FR highlights inclusion of CCs for integrating with PHC but then the urban floating communities will be left behind without planning to have linking with urban PHC hub; where are those? The FR should identify those urban PHC and plan accordingly.  Public private partnership for urban PHC hub to be explored. 

The Vaccine trails and research for vaccine to be focused duly. 

TB/HIV integration to be more emphasized on the priority districts (26) for HIV. Joint integrated initiative could be proposed. Similarly, joint integrated actions to be designed at Malaria prone districts (missing links). 

For HIV-

The target for reached KPs is not satisfactory according to the latest size estimation study used. For MSM/MSW the most vulnerable one but it reached targeted 50% according to the national size estimation though priority districts (26) proposed higher; it means 50% will be left behind, who are crucial for halting infection. FR should have be adequate thought on it. 

FSW targeted reached is too low and even lower than GC7 is not acceptable. Virtual reached could have planned and public private partnership (national pharmaceuticals for condom and lubricants can be discussed), CSR to be planned for reaching FSW district wise. 

At this stage of transition from donor funding to self-sustaining funding more innovations to be brought in both the FRs using domestic capacities, domestic private and corporate sector.

Thank you for sharing the latest draft of the Funding Request for all 3 disease programs.

Overall, the draft is progressing well, and the narrative is generally strong. That said, before the next revision, I would encourage close attention to a few priority issues that are important for the overall quality and coherence of the FR and core documents. The feedback should also serve as a reference for both TWG and OC when reviewing and recommending the FR drafts in the coming days.

Ensure full consistency across the package — the narrative, budgets, PF/PGT, gap tables, and implementation arrangements should align throughout. In a few places, there are still mismatches in figures, structures, and assumptions.

Strengthen the transition and integration narrative — the request should more clearly show the transition pathway by year, including how integration into government systems and PHC will happen, and how NGO roles are expected to evolve over time.

Revisit prioritization and budget realism — in some areas, the scope still appears too broad relative to available resources. This is particularly important where the draft suggests expansion rather than a more focused transition or maintenance approach, for both HIV and TB grants.

Avoid duplication with domestic and partner financing — commodities, HR, and operational costs already covered under the DPP or by other partners should be clearly mapped and excluded from the Funding Request.

Budget split recommendations: 

TB budget split (NTP and BRAC)

• Recommend aligning broadly with the GC7 split ratio
• Maintain approx. 54–55% (NTP) / 46–45% (BRAC)
• Avoid excessive stockpiling of health products; ensure procurement reflects realistic program needs
• BRAC should gradually plan reduction of CHW budget for transition and the budget of year 3 should be substantially reduced to reflect the integration and transition of CHW/HRH back to government system
• HIV transition and budget alignment
• Transition of key prevention services not yet clearly reflected in budget and PF
• Recommend substantial reduction of ICCDRB Year 3 budget to reflect the integration and transition of prevention, referral, care and other key services back to government system
• Increase NASP share accordingly to reflect phased absorption and sustainability
• Adherence to allocation ceiling
• All disease component budgets should remain within the allocation ceiling
• Additional activities — after prioritization — should be moved to PAAR, not included within the allocation

Addressing these points will help ensure the revised draft is more coherent, strategic, and aligned with GC8 expectations. 

Please also refer to the detailed comments in the attachments for specific suggested revisions.

HIV & Malaria funding

 

As briefed by the BCCM Secretariat, the Honorable Chair of the BCCM (the Minister of MOHFW) and the State Minister, including all respected members of the BCCM, listened to the FR presentations for the three diseases and have approved the funding splits for Malaria and HIV.

 

Regarding HIV

• Strategic allocation: GoB HIV PR (NASC’s) allocation has been doubled in GC-8 compared to GC-7. Comparatively, a higher proportion of funding was allocated to the single NGO PR (i.e., icddr,b) to address the prevention of the bulk coverage of the key populations across 33 districts, address stigma and other critical needs for strengthening health and community systems for the key population, and to ensure the community’s readiness and engagement. This approach was discussed in the meeting when you and the CT team met the DG at his office. The proportion of NASC cannot be increased in the last year of GC-8, as that indicates NASC will continue to rely on GF funds, which goes against the philosophy of integration. Rather, what we want to see is that government investment is increasing over the years, indicating the ownership and a sustainable transition of KP services to the GoB. The Government investment and actions should be planned based on the evidence and experiences of integration obtained in GC-8. After completion of integration in Year 3 of GC-8, NASC will maintain all integrated sites in GC-9, and icddr,b will render their TA support and policy advocacy. Thus, NASC will get the major percentage of GF funds in GC-9 (the last cycle).
• Integration, transition and sustainability: DGHS and icddr,b are currently co-designing the integration and transition strategy. We will send you a separate write-up when ready. Given that Global Fund support for HIV will be stopped after GC-9, we must prioritize and ensure that both the health and community systems are ready to serve each other. Any abrupt integration without any well-planned activities would jeopardize the smooth transition and sustainability of KP services. We must also capacitate NASC so that from GC-9, they can operate the HIV interventions from the GoB health facilities with a contractual agreement with the KP communities. And at the end of GC-9, the government staff of NASC should operate the HIV interventions. We should not be in a hurry in GC-8, which may jeopardize the integration and the resultant transition and achievement of Bangladesh (i.e., keeping the HIV prevalence low compared to many other countries).
• National ownership: The Funding Request (FR) must reflect national priorities, commitment, and ownership by the GoB in the transition phase of HIV and Malaria.

 

TB grant split

As a consensus was not reached between the NTP and BRAC before the last BCCM meeting, the Honorable Minister and the State Minister have directed a split of 60% for NTP and 40% for BRAC. The BCCM meeting decided that:

• The Technical Committee of TB will meet and review this further and report back to the Honorable Minister. Both parties must now work with the Technical Committee for TB to reach a final consensus. I will personally oversee this process.

I will sit with the FR writing committees to address your remaining feedback on different diseases.

Greetings from UNODC Bangladesh! Thanks for sharing your feedback which is very much helpful and praiseworthy! I am also agreeing with Professor Dr Foara Tasmim, Additional Director General (Development and Planning) and I have few comments/suggestions mentioned below.

 

1. Transitioning towards sustainability means that interventions need to be supported with government funds, not with the Global Fund. And this should be from all sectors, including Ministry of Home Affairs.
2.  In case of health revenue allocation from the current DPP for targeted prevention interventions for HIV needs to be increased, that is what the Global Fund should advocate for.
3. GC8 should focus on evidence-based readiness of health and home affairs sectors (like Prison, Dept. of Narcotics Control and Bangladesh Police).
4. Global Fund should also support government in addressing social contracting and engaging grass-root level Community Led Organizations (CLOs) through the 2025 PPR, by engaging a good procurement expert.
5. A sudden shift of GF funds to NASC will not have a significant impact in transition and sustainability. Rather focus on health sector readiness, prison transition, and local CLO rights

Thank you for the prompt feedback. We have carefully noted the guidance and approaches from the Funding Development Committee, as outlined below. Should it be decided to follow such direction by the MoH, it would be helpful for these decisions to be clearly explained and well justified in the Funding Request Narrative, including how they relate to transition planning across GC8 and GC9.

The CT feedback is intended as a constructive input to support the country in further strengthening the quality of the Funding Request and aligning it with the GC8 strategic shift and priority requirements, and support the government Health priority for strengthening PHC services. We look forward to receiving the updated version of the Funding Request.
