Bug Report - PACA ClaimResponse new field

Hello dear BCDA team --

Just a heads up for your awareness that will need to be fixed:

ClaimResponse Resources being returned by the BCDA endpoint now include the ClaimResponse.item[] field (previously the item[] array was included only in the Claim Resource).  However, the FHIR R4 specification has a required field "itemSequence" within ClaimResponse.item and this is not being included.  This element will need to be added or the Resources will be rejected by downstream systems unless inferring/inserting a placeholder value which ofc is not correct.

Please let me know if any questions/clarifications would be useful in the fix process.
