EOB vs CCLF - Linking Part A Claims

Hi,

As suggested in a conversation from 2024, I am linking EOB and CCLF part A records using natural keys - 

• For Part A files (CCLF1, CCLF2, CCLF3, and CCLF4):
  • PRVDR_OSCAR_NUM (Provider OSCAR Number)
  • BENE_MBI_ID (Medicare Beneficiary Identifier)
  • CLM_FROM_DT (Claim From Date)
  • CLM_THRU_DT (Claim Thru Date)

I am finding instances where instead of matching the EOB records by provider oscar number, I can match by billing oscar number, BLG_PRVDR_OSCAR_NUM, as well as by billing NPI, FAC_PRVDR_NPI_NUM. What would be an explanation for this disconnect and it appropriate to matching using these other identifiers if there is no match using provider oscar number?

Thank you for your questions related to linking EOB and CCLFA records on natural keys.

These facility and billing provider numbers have a different role in the processing of a claim. According to the CCLF Information packet, PRVDR_OSCAR_NUM is the facility's CCN, formerly known as the OSCAR Number, which is the identifier used by Medicare/Medicaid for certification of the type of services a facility/provider can provide in the claim. This is different from the standard NPI (National Provider Identifier), which is a unique 10 digit number that identifies providers in claims, referrals, etc.

The billing versions of these facility and provider numbers and fields are often the same as the facilities or providers that are providing the services in the claim. However, there are cases where the provider who actually submits the claim (the "billing provider") could be different from the facility where services are provided or the location of care, like if a provider associated with one CCN has the ability to bill from different locations (like at a hospital compared to a clinic) or for different services under separate billing provider numbers. Although the billing provider could act as a proxy where the facility number is not available, it would be more accurate to use the natural keys outlined in the information packet to combine the EOB and CCLFA records.

There seems to be a disconnect in the various documentation available or in my understanding on the Part A natural keys.

According to https://www.cms.gov/files/document/cclf-information-packet.pdf,  the natural keys for Part A claims are • CLM_BLG_PRVDR_OSCAR_NUM  • CLM_FROM_DT • CLM_THRU_DT • Most Recent MBI (MR_MBI) .

The CCLF data has CLM_BLG_PRVDR_ OSCAR_NUM and PRVDR_OSCAR_NUM and sometimes these can be different. 

Based on the BCDA data dictionary at https://bcda.cms.gov/assets/downloads/BCDA_Data_Dictionary.xlsx, the oscar number in the EOB data is PRVDR_OSCAR_NUM. Which oscar number in the CCLFs does this tie to - CLM_BLG_PRVDR_ OSCAR_NUM or PRVDR_OSCAR_NUM?
