Should we have a section on ACA and how Bayada will be ahead of the pack by having a transitionalist?

Under the Medicare shared savings program

(Section 3022), moreover, accountable care or-

ganizations will be required to submit perfor-

mance data that may address care transitions

across health care settings. Health homes (Sec-

tion 2703) are designed to provide comprehen-

sive care management, including transitional

care, to people with chronic conditions. The na-

tional pilot program on payment bundling (Sec-

tion 3023) will test an integrated, episode-based

payment and care delivery model, including ser-

vices such as transitional care.

I think this would be valuable to add to the Long Term levers slide - a note that we're not likely to be the only folks in the home health industry jumping into this exact space with a physician...
