I would like to state that as per policy terms with bank 100% of medical treatment bill should be sanctioned, and GIPSA package rate subsequently agreed is not applicable for  Medical Insurance Scheme  for Retired Bank Employees.

Under this Medical Insurance Scheme I have already been sanction Rs. 37000.00 claim under cashless scheme for  treatment of CATARACT RIGHT EYE at Rajas Eye Hospital, Indore vide claim No. 12510187 dt 7.4.2016. A copy of your sanction is sent attached herewith for your perusal.

I therefore request you to kindly sanction & release the differential amount of Rajas Eye Hospital bill Rs. 13000.00 to my account No. 2547257010237 at Canara Bank SME Br,., Indore under advice to me.
