For Information and kind attention of of Corporation Bank Retirees:

                                                                          IBA Group Medical Insurance Scheme for Retired Employees – Issuance of Premium Paid Certificate for claiming benefits under 80D of I T Act.

                                    The premium debited to account of such retired employees are eligible for claiming benefits under Section 80D of the IT Act.  The premium in respect of the retired employees was paid by the Bank directly to the insurer by debit to their respective accounts. Hence, the insurer will not be issuing any receipts/acknowledgement for premium paid to the retired employees for the purpose of claiming relief under Section 80D of Income Tax act.
                                    Corporation Bank retirees may contact the concerned branches,   to  obtain  certificate,  whose accounts have been debited with Premium  as per the format provided under Retirees Corner.   http://www.corpbank.com/node/126775.

 I am a retired staff of Syndicate Bank, 86 years of age and my wife is 74 years old. Should one be hospitalized how do I make a claim if I have no ID or any identification that I am a member of IBA Group Medical Insurance Scheme

Dear Mr. Vera Cardoso,

                                            1.In order to be eligible for Cash Less treatment for yourself or your Spouse , in any net work hospitals under IBAs Medi Claim Insurance ,you should be

a member of the scheme,by making payment of premium as per instructions given by your Bank(ie:Syndicate Bank).Bank will provide you all details in this regard.

                                            2.For your information;  Group Medi Claim Insurance Policy (Retirees) bearing No:500100/48/15/41/00000521 for the period from 2.11.2015 to 31.10.2016 was issued by United India Insurance Co,in the name of IBA a/c Syndicate Bank.

                                            3.If you are a member of the above scheme,  TPA (Third Party Administartor)of the United India Insurance company might have provided you a personalised  ID card (E-Card)in your name and spouse along with detailed guidelines on claim. Such ID card to be produced ( along with any Photo ID Card)to net work hospital of TPA  while seeking admission for treatment (under planned or otherwise).TPA for United India Insurance Co for Syndicate Bank retirees is;VIDAL HEALTH TPA PVT LTD.

                                            4.Cashless treatment is advisable since the bills will be settled by the TPA on prior approval.Reimbursement of claim can be made asper instructions given by your Bank.

Dear Sir,
My account with Syndicate Bank,Margao GOA (a/c72072020001742) has been debited with Rs,7,494.00 for Mediclaim Insurance.
Please guide me how do I get my ID Card, For your information I an 86 years old .The only information the bank gave me was to visit the sight on the net.

Madam,
                             Since you are already covered under a Health Insurance Policy administered by TPA ( In your case Vidal Health TPA P Ltd) of United India Insurance Co,, you will be issued a Vidal Health TPA ID card. If your health insurance cover is issued through your Bank (Syndicate Bank), you may not be issued a physical ID card some time but you may have a E-card. In general all TPA s have provided for downloading E card from their site for retired staff covered under IBA Medi Claim Policy.Few banks have sent Pfysical ID card with a kit containing details on Cashless Hospitalisation/Planned admission to hospitals with further details on net work hospitals list etc.
                             I can understand the problem with super senior retirees in getting details from Web Sites of Bank or TPA and kindly try to get in touch with your pension paying branch or your `friends in
bank so as to get the E card downloaded for your self as well as for your spouse. This is the basic thing you may require for treatment in hospitals.

For syndicate Bank, you may ring up Toll free no-18004259510 or Mumbai no 022 29314700.
Tell them your name, Bank's Name and Employee no.
Ask them for ID and they will email it to your email address.
