E card - pnb retirees

I am a pnb retiree and opted for IBA medical insurance policy for 2024-25. Can anyone guide me how to download e-card.

1.You may go to following link:

http://www.heritagehealthtpa.in/HeritageHealthTPA/HOME/EcardDownload.aspx

2.Select "IBA-National Insurance Ltd" from dropdown list

3.Under corporate mention PNB from list

4.Your PF No 

Card can be downloaded then.

Please go to the website of Heritage health TPA

Click menu download E card

Chooswinsurance company name As National insurance IBA policy 

Next colum. You choose corporate name Punjab National Bank 

Next column. Employee ID 

Enter your SPF number 

Click submit 

Allow cookies . As some times popups may be not allowed as you might have made the settings 

Please download and save or print if you have printer

Your pf no. should have 7 numbers. If not add 0 before your pf no. and make the numbers in 7.
