Information and consolidated data regarding Medical Insurance Scheme for Retirees and Spouse of Deceased Employees of Dena Bank

As per the Circular No. 265/47/2015-16 dated 21.11.2015 issued by HRM Department regarding Medical Insurance Scheme for Retirees and Spouse of Deceased Employees of Dena Bank, the Medical Insurance Scheme introduced by IBA shall  become effective from 30.03.2016.
 
The insurance premium to be paid by Retiree / Spouse of deceased employee is as follows.

FOR RETIRED OFFICERS
Prorata Premium			Rs. 3,879
Service Tax @ 14%			Rs. 543
Swachh Bharat cess @ 0.5%			Rs. 19
		Gross Prorata Premium Payable	Rs. 4,442
FOR RETIRED AWARD STAFF
Prorata Premium			Rs. 2,910
Service Tax @ 14%			Rs. 407
Swachh Bharat cess @ 0.5%			Rs. 15
		Gross Prorata Premium Payable	Rs. 3,331

It has proposed to recover the premium amount from their Saving Account / Pension Account ( mentioned on Option cum Consent Form) on 28.03.2016.
 
In view of the above, all the Retirees and Spouse of Deceased Employees of Dena Bank  are advised to keep sufficient balance in their Saving Account / Pension Account ( mentioned on Option cum Consent Form) before 28.03.2016 so that premium amount can be recovered and remitted to Insurance Company.
 
Employees who retired and spouse of employees who have expired  between 30.03.2015 and 30.09.2015 , are also required to submit Option cum Consent form to  switch over to the new Medical Insurance Scheme for retirees w.e.f.30.03.2016 by paying applicable premium to insure self and spouse only otherwise insurance cover will not be available to them.

Employees who retired after 01.10.2015 shall continue to be covered under the existing IBA Medical Insurance Scheme for staff members along with their dependents upto 30.09.2016 and they are required to switch over to the scheme of retirees w.e.f. 01.10.2016.

We are attaching herewith the consolidated statement for the Option cum Consent Forms of Retired Employees / Spouse of Deceased Employees of Dena Bank received from various Zones  for enrolling in New Medical Insurance Scheme of IBA. All eligible Retired Employees / Spouse of Deceased Employees of Dena Bank who submitted their Option cum Consent Forms are requested to verify their details available in consolidated statement. If any discrepancy found, they may contact us by email on  ibamed...@denabank.co.in.
