Mr Partha Sarathy, 

For UBI and PNB retirees Heritage Health is the TPA. 

Insurance co may be selected as " IBA- National Insurance" under scroll down menu. 

Rest is name of your bank as above and

Employee ID

You may be able to down load the card.

What is Corporate Name ? By entering Corporate as Punjab National Bank, it says no such matching record found.

Corporate name under Heritage Card site is your banks name. 

Scroll down Menu may show related banks name. ( Meant for three banks only. UBI, PNB & J& K Bank)

Please click IBA national insurance 

Corporate name :punjab national bank 

Employee ID: your spf #

At present  only staff pensioner health card is only available for download

Spouse health card is not available. 

As per information  PNB had not furnished details of spouse to TPA ( a serious lapse)

I understand by Monday we can view spouse health card too

Dear Mr Mohan,

1) I understand for canara bank, Raksha is the TPA. As such canara bank staff will not be able to download e card from this link.

2) I feel policy has not yet been renewed.  

3 ) Only after the policy is renewed and records updated , probably we would be able to download e cards.

4) We may have to wait for about  week.
