My name is Dr. Nasim. I am a physician in the Atlanta area.

 

I am writing to let you know that we, the physicians in Atlanta, are arranging a Health Fair for the benefit of the Bangladeshi  community. We will initially do a blood draw on September 23^rd, Sunday and 2 weeks after this, on October 7^th, Sunday the lab result will be reviewed with a face-to-face patient evaluation. The doctors visit is entirely free of charge as the they have decided to offer voluntary service. The lab tests are offered at a highly discounted rate of $30 per person only. These tests at a outside lab will cost close to $300-350 per person. We hope that the members of our community will take advantage of this excellent opportunity without delay.

 

I am attaching a copy of the flyer and a health history form for  the Bangladeshi community.

We request the patients to complete the health history form prior to showing up for lab test on September 23^rd to reduce waiting time.
