It would be good to see head/tail lights with (provision to put in) a chord/strap, which can be looped around the handlebar or seatpost; that will act as a backup, in case the mount fails. Few headlamps have that, though I think it's primarily meant as a wrist strap, when carrying the light in the hand.

Happened a few times with my phone mount as well. I now use a plastic tie as backup to keep the mount from falling off.

I've attached pictures of the broken mount. Most of online reviews say that this is a great light till you loose it. Seems like this is a common problem and niterider (in some post online) supposedly has changed the material used for the mount.
The first tail light that I had worked for 4 years. I'm wondering if I should choose the rubber strap mount option next .
Any suggestions?

I should add... I have had the tail light too pop out few times, from the tail light strap on the BOTS wedgie. Luckily, I heard it pop each time, so I stopped & picked up the tail light. Now I use a chord to tie the tail light to the wedgie.
I am not sure if this issue is specific to the BSA tail light, or the BOTS wedgie. My guess is that any tail light fixed on a strap, may run the risk of popping, because it shakes around while riding.
