Urgent: Club members need to complete the waiver ASAP

Dear All,

 

Please see the email previously sent by Max, the assistant director for sport clubs. Every club member will need to complete the waiver online ASAP, if you have not done so.

 

Please follow step by step below:

 

1)      Go to http://www.orgsync.com, click on “Login”, then select University of North Carolina @ Charlotte.

2)      Login with you Niner Net Username and password.

3)      Select our club – “Badminon Club” from the drop down menu of “Organizations”.

4)      Click on “Forms” from the list on the left of the screen.

5)      Click on “Sport Club Member Contact Information & Release and Indemnity Agreement” to select the form.

6)      Click on “Begin Form”, then fill your info (Name, DoB, Address, Insurance, etc.) according to the guide.

7)      Last, submit the form (for those have multiple clubs, make sure which club you are submitting on behave of).

 

Again, I would like to emphasize that this waiver is urgent. Those who do not complete their waiver will be asked to leave practice next week, as claimed by Max.
