Hi everyone,

Signing the Waiver form is very important. This is the policy of our school and I hope everyone can sign it before start practicing. 

Here is some steps for signing the form: 

1. go to  https://orgsync.com/login/university-of-north-carolina-charlotte 

2. use you 49er account to log in

3 . search sport participation waiver 2015

 4. fill out the form. 

You need to provide your health insurance information ( you can screenshot of your electronic insurance card or take pictures of your physical  card)
