Mandatory 2023 Farm Registration Questionnaire

Good Afternoon Produce Industry Stakeholders,

 
The Arizona Department of Agriculture is collecting the Mandatory 2023 Farm Registration. 

The farm registration helps improve food safety throughout the Arizona produce industry.
 
The required farm registration is outlined in section A.R.S. § 3-525.03(c) and 

must be submitted annually to the Associate Director by October 1st of each year. 

If there are changes to the information submitted in the questionnaire, then the owner, 

operator, lessee or designated representative of the farm shall notify the Department within 

60 days after the change. 
 
Attached is a letter from the CFV Associate Director describing the program requirements 

as well as the new 2023 Farm Registration. 

The 2023 Farm Registration must be submitted to the Department no later than October 1^st of this year.
 
The online form was designed for your convenience. The online format can easily be submitted 

within a few minutes using Google Forms.
