v1 Lookup API Fixes

FYI, we've just pushed some fixes to GET create.awe.sm/url.:format

* list of urls is once again being properly filtered when clicked=true and details=true
* default pagination is back to per_page=100 as documented
* properly displays all-time click counts when start_date and end_date are left blank

Everything else should remain the same. If you see any other changes or experience any issues with this endpoint, please let us know ASAP.
