Offset with distance gives incorrect result

Does the problem occur a the "tips" or "points" or is it a constant
throughout? We have run into problems where the object (usually a
polyline) juts out. In our case the distances is usually greater.

It's very likely that "snap" is on and it is set to 1 (e.g. mm).
Personally I never use snap only object snap :-)

Very good point Chris.

As the interior angle decreases, the distance between offset endpoints
increases. However in parallel sections the perpendicular distance 'should'
always be 100. For example with a 100 unit offset.

135° interior angle = 108.35
45° interior angle = 261.60

So the most accurate representation would be achieved if you filleted the
resulting offset to 100, making all points along the path truly 100 usings
away.
