From what I have found, there is no manual that covers both. There are
manuals for C++ (syntax, keywords, tutorials, etc...), and there are manuals
for ObjectARX/DBX which is AutoCAD's API for C++.

For beginners I would highly recommend "Visual C++ 6.0 for Dummies" to start
off. For more advanced techniques I would look into classes offered at the
nearest community college.

As for Object ARX/DBX, I would look at the tutorial provided with the SDK.
Believe it or not, I have found AutoDesk's documentation on customization to
be rather thorough and helpfull, so long as you are familiar with the
language that is (with the notable exception of LISP, which AutoDesk
provides a very good tutorial.)

Hope it alteast gives you a pointer in the right direction.
