[patch 6/6] implement mtime checking in HtPasswdChecker

With this patch HtPasswdChecker will check every time before authenticating
a user if the password file was modified since the last read. If so, the
user database is read again - you can make modifications on the fly without
restarting your app.

I think this checking causes no visible performance hit because the OS
should cache filesystem data if there are no changes (as long as you use
noatime to avoid writes).
