Transform missing

Hi All, 

What do I do to fix this every time QGIS loads a project ?

"No Transform is available between GDA94 / MGA zone 56 and EPSG:4326 - WGS 84"

Which QGIS version are you using? 

Does it do this when loading a particular project?
