1) Our intake form/patient summary
2) The Cognitive Acuity Test is a Geriatric Depression Screen questionnaire that helps us answer the cognitive impairments/depression screening part of the visit
3). The USPTF screening checklist is a guide, our PA (who does the visits) just goes to the USPTF site and in a minute can generate a more specific one for the patient.
4) We left the Advanced Care Planning question in because it's good for us to know.
5) We also have available a blog that we let patients read so they can understand what the visit is and isn't.
What we do is use a specific template that has a section for vitals, pulls their medication list, current provider list. The PA has a discussion with the Patient, then we just scan the summary into their chart.
Athena is currently working on incorporating aspects of this and the visit template I created in Athena. I just talked to them last week about what we do. They will probably have something by the end of the year if not earlier.
Although at first patients didn't understand why we were having them come in and stuff, everyone has liked it and it's given us a chance to catch up with the health of our medicare population and get paid for it.
