I have attached the visit notes from DOS 2/2 for your review. In reviewing the notes I found the following:
Notes: Patient presents to the clinic with complaints of lower back pain and neck pain that beganafter involvement in an MVC on 12/25/25. Her cervical MRI showed no significant findings. Herlumbar MRI showed a mild annular tear at L5-S1. After review of all imaging, no significantfindings are noted. I recommend continuation of conservative treatment, including physical therapy.If symptoms worsen, epidural steroid injections and chronic pain management are recommended forcontinued symptom management. No surgical interventions are recommended at this time. She willfollow up as needed.
