Hi Joe,

Attached are the student evaluation forms. Can you provide me with the same info you supplied for the Fall 2013 ASAM grades. I think I have sent this to the class but if not can you make sure they receive it?

IMPORTANT: Please have the ASAM class return this form no later than Wednesday, June 11 so I can submit ASAM Grades for Spring quarter.
