Hello Members of Arusha -UDEC group.

I trust this mail will find you fine, 

UDBS is making follow up to evaluate  for her TOT's training and seminars  conducted since 2008 on different aspect to date , being one of member group you attended, your requested to fill up the attached questionnaire and send it back to me.

for those employee participated they should share the Employee questionnaire attached to be filed up by the employer 

kindly note that this is important information need by UDBS for you as a consultant and for UDBS management.

NB. for those who have been already visted by UDBS staff and done this exercise please ignore this information.
