predefined streams and subasins in watershed delineation

I am trying to use predefined streams and subasins for use in the initial watershed delineation for a SWAT project.  I found that the attribute table for the subbasins needs to contain only the following information: PolygonID, Area, Subbasin (#).  For streams it is: WSNO, LINKNO; with option DSLINKNO, Length, and Drop (Where thee WSNO and LINKNO appear to be numbers that should correspond to the subbasin number that the reach is located in).

When I try to upload the files however, I get the message that I dont have the correct fields included.  I am wondering if anyone can help direct me to any official documentation regarding the formatting for the files used here, or can give me any guidance on the formatting of the files.

Hello,
To use this option, you need to make sure that the fields are EXACTLY like the ones in the example, meaning the NAME, Format, Length, and in the EXACT same order. Then it works :)

Grace where you able to prepare the watershed and stream reach using that template? I need help with that also.

I created my subbasin and stream vector file just like the example inputs of ArcSWAT and it worked. You can find it under Datbases-> ExInputs.
required fields for stream are just ARCID, GRID_CODE, FROM_NODE, TO_NODE, subbasin, Subbasin R.
for subbasin you only need GRIDCODE, Subbasin

When you install ArcSWAT, you get examples and a manual. You should be able to find it in the folder where you installed ArcSWAT (C:\SWAT\ArcSWAT\ArcSWATHelp\ArcSWAT_Documentation_2012.pdf). In the manual you might want to look at page 13, where User-Defined Watersheds are explained. Examples are mentioned over there. The examples are found in this directory: C:\SWAT\ArcSWAT\Databases\ExInputs. There are two shape files, one for UserStreams and UserWatersheds.
Mimic the fields of the shape files (as Aida mentioned).
