The same happens for me too. I guess I just never looked at the toolbar
before adding data before. Actually the icon is still there, but in a
greyed out disabled state, but for some reason the black lines are shown
in the same grey as the background instead of a darker inverse. I
classify that as a usability bug, but am not sure who is responsible,
Arcmap or mapbook.
