New Template in KB Gallery - Volunteer Exit Interview Form

Crew Resources Team Lead Sally has completed another KB template template for ExCom and Team Leads use - Volunteer Exit Interview Form - to be completed when a volunteer is stepping down. The idea is to get feedback on how to improve the role and KB's funcitionality as we grow. 

This template is to be used by the relevant department Facilitator, co-lead, or 2iC when a volunteer is existing their role.
